Healthy Food Healthy You is a transitional meal program designed to assist those recently discharged from a hospital who could benefit from home delivered meals.  These meals will provide good nutrition and aid in recovery after illness or surgery in hopes to reduce chances for future hospitalizations.

 

Eligibility:

  • 60+ years old

  • Live in Allen County

  • Discharged from hospital or facility within the past 30 days

  • Lack other meal support service in home/community

  • Unable to prepare nutritious meals (PB& J, can of soup, crackers & cheese do not count)

  • Not enrolled on Medicaid (if pending for Medicaid Waiver; can still receive a voucher until enrolled)

Services:

  • Meals will be provided by SimplyEZ Meals.

  • Up to 14 meals per week, for 4 weeks post-discharge.

  • Quantities can be adjusted to meet needs, but can not surpass the totals above.

  • Please note, these services are designed to be short-term and will not surpass 4 weeks.  If there is a long-term need, individuals may still be issued a voucher to receive meals through HFHY, but a referral should be made for traditional Home Delivered Meals.  An assessment will then be scheduled, and the individual will be added to our HDM waiting list. 

 

If you know of an individual who could benefit from the program, please contact the Nutrition Team at nutrition@psa3.org.  Vouchers will be monitored by the Nutrition Team and distributed directly to eligible individuals or to the Care Manager to forward on to the individual.  Once the eligible individual receives a voucher, they will then reach out to SimplyEZ Meals to coordinate the start of services.  Click here for an example of the voucher!

  • Unable to prepare or acquire an adequate meal 

  • Lives alone (special consideration for couples with sufficient need) 

  • Not currently receiving in-home services from another source that can provide meals

DISH - Dining to Improve socialization & Health

The DISH Program allows individuals a different way to receive a nutritional meal, socialization, and nutrition/health information.

  • Nutritious meals in a restaurant environment 

  • Menu options meet nutrition program guidelines 

  • Beneficial socialization for older adults

Director Comment:

We all enjoy a meal out once in a while and it’s exciting we offer a program that enables older adults to have this option. The DISH program is an innovative way for local business owners to support a program that provides older adults in our community the opportunity to enjoy a meal out a few times a month.

It’s important to us that individuals have a variety of restaurants to choose from and would welcome hearing from local restaurant owners interested in joining our program!

 

Provider Comment:

Our customers that are using the vouchers from the Senior Dining Program absolutely love coming to our restaurant in Ottawa. They are very happy with the many choices on the menu that they can choose from and are delighted that they can come anytime of the day. They like the fact they can choose from breakfast, lunch or dinner seven days a week. Some folks have told our staff that they wouldn’t be able to eat out if it wasn’t for the senior dining program.

We love the fact that we are able to make these folks happy.

Senior Farmers' Market Nutrition
Senior Farmers' Mkt.

The Senior Farmers Market Nutrition Program is found in 45 of Ohio's 88 counties.  It is funded by the US Department of Agriculture.  Eligible individuals receive $50 of $ coupons to redeem for fresh produce and honey at participating farmers markets and farm stands.  Eligibility:  Age 60 or older and meet income guidelines of 185% or less of poverty level.
