Care Coordinator

Responsibilities: Identify and assess victim service needs and commission a range of services to support identified needs, develop and maintain collaborative relationships with other community agencies, and assist to develop a professional counseling program for the Area Agency on Aging 3 for the purpose of assisting older adults with depression, anxiety, grief, trauma, role transitions, stress, and health concerns.

Qualifications: Licensed Independent Social Worker (LISW) or a Licensed Independent Social Worker, Supervisor (LISW-S) required. Preferably one-year experience in Long Term Care/Home and Community Based Services for elderly and/or disabled individuals. Good oral and written communication skills with strong computer proficiency. Applicant must have a valid driver's license and automobile insurance.

Ohio Home Care Manager

Responsibilities include but not limited to develops/coordinates/manages care plan for services for all ages. Advocates for services and benefits, conduct family/service provider case conferences, arrange for service delivery and reassess needs and services for care.

 

Qualifications: RN or LSW required, 1 years prior experience in community health care, general knowledge of Medicare/Medicaid and community resources needed. Great communication skills, ability to multi task, meet deadlines, be organized and detail oriented in a fast pace environment and computer experience is a must. Applicant must have the ability to telecommute and have a valid driver’s license with auto insurance.

Health Educator/ Coordinator

DEFINITION: This position will coordinate health education activities related to healthy aging and disease prevention programs. The Health Educator will assist in developing and implementing health education outreach and engagement strategies, health education materials and group education sessions. The incumbent for this position will play a key role in planning, developing, and implementing health education training and prevention programs, as well as evaluating training strategies and materials. A primary function of this position is to serve as a health communication content expert and advisor related to healthy aging and other health related issues.

 

MINIMUM QUALIFICATIONS:

• Bachelor’s Degree in Health Education, Public Health, Sociology, Social Work preferred but not required.

• Highly effective communicator, both verbally and in writing.

• Energetic, enthusiastic, excellent organizational and leadership skills, and the ability to work with minimal supervision.

• Experience working with diverse and/or underserved populations (as employee or volunteer); and/or working with community-based organizations, public health agencies, and/or health care facilities.

• Excellent public speaking skills.

• Knowledge of general health promotion topics and primary and secondary prevention of specific chronic conditions such as diabetes, heart disease, obesity, and others.

• Ability to manage multiple tasks simultaneously, to stay organized, and to prioritize work appropriately.

• Proficient in computer skills. Intermediate competency in Microsoft Office Suite programs (Word, Excel, PowerPoint; Access), internet browsers, and email programs.

• Must have a valid driver’s license, access to an automobile, and automobile insurance meeting the minimum requirements of state law.

• Final candidate must agree to a BCII Criminal Records Check, which includes fingerprinting and pre-employment Drug Screen at the expense of Area Agency on Aging 3.

• It is the sole responsibility of the employee to keep up with all required licensure (Examples RN, LSW, RD), including continued licensure renewals throughout your employment with Area Agency on Aging 3.

DESIRED QUALIFICATIONS:

ESSENTIAL FUNCTIONS:

• Develops and provides education and outreach on health aging and chronic disease prevention topics in collaboration with community partners.

• Delivers health education and outreach programs at various community venues.

• Conducts periodic assessments of community interests and health education needs in areas of health promotion, disease prevention and maintenance, in collaboration with community and academic partners.

• Develops, adapts, and implements education materials and presentations to be culturally appropriate for specific populations.

• Establishes and maintains rapport and strong working relationships with community partners, attends community meetings at various times.

• Collects and maintains program monitoring and evaluation data on outreach and education activities, and prepares periodic reports of outreach and education activities.

• Recruits, trains and coordinates Lay Trainers and Coaches for the Healthy U, Matter of Balance, and other agency education programs. • Develops, implements, and coordinates various health education activities involved in providing national and local information and educational programs for Area Agency on Aging 3.

• Adapt and implement national and local information, campaigns and education programs to meet the needs of high-risk populations in the service area.

• Maintain interface with civic groups and health service agencies; develop health goals and determine availability of health services for cancer patients.

• Prepare reports and analyze adverse trends and appropriate recommendations or conclusions; maintain accurate records reflecting work activities.

• Ensures the fidelity of evidenced based programming and conducts fidelity checks of Lay Leaders as able.

• Work/job performance location and scheduled hours will be determined by immediate Supervisor/Vice President/Chief Operations Officer/Chief Executive Officer.

• The position is directly responsible to the Director of Wellness and Support Services/VP of Community Living Services/Chief Operations Officer/Chief Executive Officer.

• Performs additional tasks as assigned by the Director of Wellness and Support Services/VP of Community Living Services/Chief Operations Officer/Chief Executive Officer.

Staff Accountant

Responsibilities include general ledger, billing/ request for funds, budgeting along with preparing financial statements and program reports.  Position also includes some payroll functions, grant compliance, audit preparation and other accounting duties as assigned.

 

Qualifications: Bachelor’s degree in accounting with 3+ years of accounting experience.  Good oral and written communication skills along with strong accounting computer skills and knowledge are required.  Applicant must have a valid driver’s license and automobile insurance.

Mobility Manager

OVERVIEW OF MOBILITY MANAGER POSITION:
The Mobility Manager works with the Ohio Department of Transportation and local partners to improve/increase transportation service for persons with disabilities and older adults by ensuring that communities coordinate transportation resources through multiple local/federal/state programs. This position will be responsible for Mobility Management which, under the Federal Transit Administration (FTA) definition: "consists of short-range
planning, management activities and projects for improving coordination among public transportation and other transportation service providers with the intent of expanding the availability of services." This position, funded by the FTA, the Ohio Department of Transportation (ODOT) is contingent upon continued future grant funding and development of local resources.

 

MINIMUM QUALIFICATIONS:
• A Bachelor's Degree from an accredited college or university in Social Work, Transportation/Urban Planning, Business/Public Administration, or Transportation Logistics preferred but not required as related experience will be considered. Two years supervisory experience or leadership preferred.
• Must have a valid driver’s license, access to an automobile, and automobile insurance meeting the minimum requirements of the state law.
• Highly Proficient in computer skills.
• Must have a telephone.
• Excellent interpersonal, written and verbal communication skills. Ability to speak before groups.
• Demonstrated ability to work independently and with others in an effective manner.
• Knowledge of the Americans with Disabilities Act (ADA) and its specific applications to public transportation. Federal Transit Administration (FTA) and Ohio Department of Transportation (ODOT) policies, procedures and practices. Knowledge of HIPPA requirements. Preferred but not required if past work experience demonstrates an ability to learn and apply regulations.
• Knowledge of the principles, procedures and strategies of coordinated human services transit-transportation planning and coordination strategies.

ESSENTIAL FUNCTIONS:
• Promotes the enhancement and facilitation of access to transportation services, including the integration and coordination of services.
• Supports local coordination with planning and policy development.
• Develop and maintain effective working relationships with all levels of staff, community leaders, government representatives, and customers.
• Responsible for locally developed Coordinated Planning as required by FTA/ODOT for ongoing funding.
• Works with VP of Aging in Place/Chief Operations Officer/Chief Executive Officer to Develop and maintain the FACTS Coalition (Future of Accessible Coordinated Transportation Services), a pro-active stakeholder group and coordinates public involvement activities.
• Develops strategies for seeking other funding sources and to leverage existing funding with non-FTA federal programs.
• Provides leadership and daily management of Find A Ride call center and related transportation staff including but not limited to consumer eligibility, referrals to provider organizations, billing, outcome measurement, fund development and other duties as assigned by the VP of Aging In Place /Chief Operations Officer/Chief Executive Officer of Area Agency on Aging 3.
• This position is directly responsible to the VP of Aging In Place.
• Adheres to consumer privacy and HIPAA regulations.

 

REQUIRED TRAININGS
• Trained in transportation computer software systems.
• Attend any training required by the VP of Community Living Services/Chief Operations Officer/Chief Executive Officer.
• New Mobility Manager training through ODOT

Volunteer Positions: Board of Trustees & Advisory Council

The Area Agency on Aging 3 is always on the lookout for members of our governing bodies.

 

Our advisory council members meet the third Wednesday, every other month at 10:00am in our agency office. They serve to represent lifestyles and cultures of each county we serve and assist us in meeting the needs of this area’s seniors.

 

The Board of Directors consists of 11 members, 1 from each of our 7 counties and 4 who are considered at-large. Appointments are for 3-year terms. The board directs the business of the agency and represents the employees and individuals we serve.
