Passport & Assisted Living Care Manager

Manage a caseload and complete home visits as required to assess program eligibility, care plans, regular contact with individuals, providers, and caregivers to determine whether services are appropriate, verification of services provided, and modifications.  This position reports to the Care Management Supervisor.

Qualifications: Current RN, LSW licensure, 1 year + in long term/home and community based services for elderly and/or disabled individuals, medical social work, geriatrics/gerontology or other substitute as approved by the Ohio Department of Aging.

Homecare Program Assessor (7-county service area) 

Conducts face-to-face assessments with individuals/family system as a basis for determining the consumer’s level of care/level of need, eligibility for available community resources and long-term care options.

 

Qualifications: Current RN, LSW licensure, 1 year + in long term/home and community-based services.

Chief Financial Officer

Directly oversees and supervises the fiscal staff, develop, and monitor budgets, track KPI’s, ROI, Audits, contracts, area plans, forecasting, grants, and all financial operations compliance with Federal and State laws.

 

Qualifications: Four-year degree in accounting, finance, or related field. 5+ years in supervisory experience, budgeting, KPI’s, contracts, grants, and flow management.

Volunteer Positions: Board of Trustees & Advisory Council

The Area Agency on Aging 3 is always on the lookout for members of our governing bodies.

Our advisory council members meet the third Wednesday, every other month at 10:00am in our agency office. They serve to represent lifestyles and cultures of each county we serve and assist us in meeting the needs of this area’s seniors.

The Board of Directors consists of 11 members, 1 from each of our 7 counties and 4 who are considered at-large. Appointments are for 3-year terms. The board directs the business of the agency and represents the employees and individuals we serve.
