Community Options Assessor - FT
Responsibilities: Daily travel will be located within Mercer/Allen County. Conducts face-to-face assessments with individuals/family system as a basis for determining the consumer’s level of care/level of need, eligibility for available community resources and long-term care options. Develops an individualized comprehensive care plan with the consumer/caregiver and arranges for service delivery and clarifies care options. This is a remote home office with daily travel to complete assessments within Hancock County. This position will report to the Community Options Supervisor.
Qualifications: Current RN, LSW licensure, 1 year + in long term/home and community-based services.

Community Living Services Care Manager-FT Exempt
This position will be a hybrid role requiring both home office and on-site work.
MINIMUM QUALIFICATIONS:

Current RN or LSW licensure (preferred) in the State of Ohio.

Skills/knowledge related to care management and documentation.

Willingness to encounter unfavorable environments, living conditions and individual physical/mental limitations to service older adults, unless physical danger is apparent.

Ability to articulate and to be sensitive in order to communicate effectively with older adults/caregivers/family members.

Willingness to remain up-to-date on other social service programs and providers in our area that serve older adults.

Ability to work cooperatively with all local agencies, organizations, groups, and public officials.

ESSENTIAL FUNCTIONS:

Analyzes the physical, mental, and social well-being of older persons and individuals with disabilities needing home and community-based services.

Manages and determines enrollment, assesses, plans care, provides ongoing communication, and contact with individual and/or caregiver(s), reassesses, and facilitates referral process for additional services, if needed.

Maintain file on program recipients and caregivers according to policies and procedures.

Assists in collection of outcomes data, satisfaction surveys, cost savings, and productivity information for reporting purposes both internal and external.

Accurately obtain and enter data in systems as required.

Assists in quality assurance activities to ensure all programs service excellence.

Develops knowledge and understanding of funding sources and rules and regulations, such as, but not limited to, accreditation standards and the Older Americans Act.

Performs as an advocate to ensure services are available that enable older adults to remain in their home and community.

Provides outreach and education as necessary to inform community of available services and resources.

Adheres to privacy and HIPAA regulations.

Program Support Specialist- FT Hourly:
Provides clerical support, documentation, and assistance to case/recovery managers and clinical staff in department. Route calls, process mail, outreach visits, track eligibility, Medicaid approvals, processing forms and applications, communicates with individuals, service providers, organizations, JFS, hospitals, nursing homes, and physicians’ offices. This position reports to the Home Care Program Support Specialist Supervisor.
Qualifications: High School Diploma or GED. Associate or Bachelor’s degree preferred. Ability to obtain and enter data in various data systems, team player, work with minimal supervision, excellent verbal, and written communication skills.

SRS Recovery Manager-Allen County
Remote home office within individual’s home visits within Allen county
MINIMUM QUALIFICATIONS:

RN or possess a Bachelor’s Degree in Social Work, Counseling, Psychology or similar field required.

A minimum of three years post degree experience working with individuals with severe and persistent mental illness (SPMI).

Demonstrate the knowledge and skills necessary to provide the level care management/assessment appropriate to the scope of program responsibilities.

Volunteer Positions: Board of Trustees & Advisory Council
The Area Agency on Aging 3 is always on the lookout for members of our governing bodies.
Our advisory council members meet the third Wednesday, every other month at 10:00am in our agency office. They serve to represent lifestyles and cultures of each county we serve and assist us in meeting the needs of this area’s seniors.
The Board of Directors consists of 11 members, 1 from each of our 7 counties and 4 who are considered at-large. Appointments are for 3-year terms. The board directs the business of the agency and represents the employees and individuals we serve.
