SRS Recovery Care Manager

Responsibilities include ensuring the individual’s involvement in decisions that affect his or her care, daily schedules, and lifestyle. Coordinates all services received by an individual and assisting the individual in gaining access to needed Medicaid State planed and 1915 (i) services, as well as medical, social, educational, and other resources , regardless of funding.

 

Qualifications: Bachelor’s degree in Social Work, Counseling, Psychology, Registered Nurse or similar field required. A minimum of three years post degree experience working with individuals with severe and persistent mental illness (SPMI) and/or a minimum of one year post degree experience working with individuals with diagnosed chronic conditions.

Ohio Home Care Manager

Responsibilities include but not limited to develops/coordinates/manages care plan for services for all ages. Advocates for services and benefits, conduct family/service provider case conferences, arrange for service delivery and reassess needs and services for care.

 

Qualifications: RN or LSW required, 1 years prior experience in community health care, general knowledge of Medicare/Medicaid and community resources needed. Great communication skills, ability to multi task, meet deadlines, be organized and detail oriented in a fast pace environment and computer experience is a must. Applicant must have the ability to telecommute and have a valid driver’s license with auto insurance.

PP/AL Case Aide

Responsibilities: include but not limited to providing dependable and reliable support and assistance to clinical staff along with accurate written and verbal communication with consumers, physicians, and providers. 

 

Qualifications: High School diploma or GED required, Associates degree preferred. Ability to data enter accurately, operate office equipment, have great communication skills, ability to multi task, meet deadlines, be organized in a fast pace environment and computer experience is a must. Applicant must have a valid driver’s license with automobile insurance and a solid past work history record.

Community Living Services Project Coordinator

MINIMUM QUALIFICATIONS:

• High School diploma or GED. Associates or Bachelor’s Degree preferred.

• Excellent verbal and written communication skills.

• Exceptional organizational skills and must be able to multitask.

• Ability to work with minimal supervision, problem solve and complete assignments efficiently and accurately.

• Ability to operate a computer to perform essential job functions including: proficient typing skills and knowledge of Microsoft Windows and Microsoft Office, primarily Word, Excel and Outlook. Power Point and Publisher a plus.

• Ability to accurately obtain and enter data in various data systems.

• Must be a team player and able to adapt to changing job duties.

• Attitude conducive to Agency Mission and goals.

• Must have a telephone.

• Final candidate must agree to a BCII Criminal Records Check, which includes fingerprinting and pre-employment Drug Screen at the expense of Area Agency on Aging 3.

 

DESIRED QUALIFICATIONS:

• Bilingual (Spanish/English) fluency.

 

ESSENTIAL FUNCTIONS:

• Works directly with the VP of Community Living Services to help ensure projects are completed on time.

• Assures all aspects of assigned projects run smoothly and efficiently.

• Collaborates with various members of the team to develop timelines and oversee projects such as but not limited to organization and presentation of division outcomes, maintenance of required federal and state data, runs and analyzes program utilization data, maintains timelines for grant compliance, assists in project coordination and follow through of large projects, and assists with timelines and development of program applications.

• Other duties would include but are not limited to assisting monthly Older Americans Act unit conversion to SAMS, exploring more efficient and potentially cost-effective projects such as program specific billing programs such as CatMatt system, and coordinate dates as well as targeted outreach for new or spotlighted programs as liaison to the agency Marketing Coordinator.

• Adheres to consumer privacy and HIPAA regulations.

• Work/job performance location and scheduled hours will be determined by immediate Supervisor/Vice President/Chief Operations Officer/Chief Executive Officer.

• This position is directly responsible to the VP of Community Living Services/Chief Operations Officer/Chief Executive Officer.

• Performs other related tasks as assigned by the VP of Community Living Services/Chief Operations Officer/Chief Executive Officer.

Volunteer Positions: Board of Trustees & Advisory Council

The Area Agency on Aging 3 is always on the lookout for members of our governing bodies.

 

Our advisory council members meet the third Wednesday, every other month at 10:00am in our agency office. They serve to represent lifestyles and cultures of each county we serve and assist us in meeting the needs of this area’s seniors.

 

The Board of Directors consists of 11 members, 1 from each of our 7 counties and 4 who are considered at-large. Appointments are for 3-year terms. The board directs the business of the agency and represents the employees and individuals we serve.
