Alzheimer's Support Program

The Alzheimer’s Support Program provides services to assist caregivers of those with Alzheimer’s Disease or related dementia in caring for the person in his/her home.  

Eligibility Requirements

• Diagnosis of Alzheimer’s or a related disorder (no age restriction)
• Meets level of care at the time of assessment

• Education and Support Groups
• Adult Day Service
• In-Home Respite
• Weekend and Vacation Respite
